Application Forms

Annuity Purchase - Annuity Purchase information and application form.

Application Forms - Forms that Members and their Employers can print, complete and send to us at the Southend office. See Contact Us page. A list of forms are below:

Application Form - requests the information required to admit a new member to The Cheviot Trust.
Application to Change Contributions - allows existing members and employers to change their contributions.
Application to Continue Membership on Changing Employer - allows existing members to continue membership on changing employers.
Expression of Wish form - allows existing members to update their wishes for the distribution of death benefits.
Nomination form - allows existing members who became members before April 1996 to update their wishes for the distribution of death benefits.
Selection of Investment Options form - allows members to direct how they wish their future contributions to be invested, switch their current funds and/or change their Target Retirement Date. Members can change their investment options at any time.
Online Account Service Form - to create an online account for you, we will require an email address, preferably your personal address and confirmation of your National Insurance number. It is very important that we have your correct National Insurance number.
